Our History
Founded in 1964 by Louis L. Boldt, Form Services, Inc. has proudly supported the Mid-Atlantic construction industry for over 60 years. Built on a foundation of quality, innovation, and customer service, we’ve grown from a formwork supplier to a comprehensive provider of concrete forming, shoring, tilt-up, and masonry solutions.
In 1999, Form Services became an Employee-Owned company, reinforcing our commitment to shared success and long-term stability. Leadership transitions over the years have carried forward our founder’s values, with Charles L. Polesovsky now serving as President and CEO.
Today, we continue to support contractors with expert service, reliable products, and a deep commitment to the people who build our communities.
